HOME INSPECTION REPORT DECLARATION PAGE

I, _______________________ am a Module 5 Student and I certify I performed this Home Inspection Report for the purposes of Module 5.

I further understand, I am using this for classroom purposes. I did not receive a fee for this inspection, and I am using it as part of the 40 hours of Inspections to receive a Module 5 Certificate in NYS.

I am certifying this for Bill Merrell exclusively in Module 5.

If I do not meet the minimum requirements, set forth by NYS guidelines, I understand I will not receive credit for this course. The Module 5 class is exclusively PASS / FAIL.  I wish to Pass the class and perform the required report writing to meet the requirements to pass Module 5.

I declare all the above is true and correct and I did not receive assistance nor did I do this report as a group project in any way.
